Description:
Since the passing of the Affordable Care Act in 2010, and the introduction of value-based care, hospitals are narrowing their list of post-acute providers and seeking out long term care facilities that not only provide quality care and have great patient satisfaction, but also those facilities understand the importance of preventing avoidable hospitalizations. This session will review proven tactics that skilled nursing and other long-term care facilities can implement to help prevent avoidable hospitalizations. Hospitals and payers are not just relying on post-acute providers to help reduce readmissions, but to proactively communicate to prevent all avoidable hospitalizations.
Objectives:
- Attendees will understand the shift in control from the hospital sector to the insurance sector and how patient referrals and admissions will be impacted across the continuum.
- Attendees will understand how the specific role of post-acute providers will be impacted in the transition to value-based care and how to adopt best practices to ensure efficiency.
- Attendees will learn to create post-acute networks and new revenue generating partnerships and programs.
- Attendees will understand how post-acute providers can best communicate with hospitals, case managers and payers to reduce avoidable hospitalizations.
Speaker Bio:
Josh Luke is a former hospital CEO, nursing home administrator and post-acute executive, well known for his expertise in transforming acute care delivery models beyond the walls of the hospital. After spending more than ten years in the health system C-suite, Luke left to write the first book on the hospital readmission penalty and founded a non-profit that teaches healthcare providers how to transform to value-based care. Luke founded the National Readmission Prevention Collaborative and is a health policy professor at the University of Southern California. He has a Ph.D. from the University of Nevada and is a Fellow in the American College of Healthcare Executives. He has authored five books, including four Amazon #1 best sellers.
