This live webinar, “Transitions of Care Through the Aging Services Continuum,” will dive deeper into an understanding of how to facilitate transitions of care in aging services and senior living communities, as well as considerations when facilitating aging in place. Best practices will be reviewed, as well as residency agreements, transitions teams, and health assessments. Case studies of situations involving transitions of care will be discussed and assessed.

Objectives
At the conclusion of this live webinar:

• Audience members will have an improved knowledge of how to understand and rebut resident arguments against level of care changes.
• Audience members will be able to identify Residency Agreement provisions that are important to level of care changes.
• Through case study reviews, audience members will learn about options on how to address residents who resist transitions, and considerations for aging in place.

Speaker Bio
Jason Lundy is a partner in Ice Miller’s Health Care Group. He concentrates his practice on long-term care. He is adept at strategically guiding providers through fast-changing and uncertain regulatory environments.
