This webinar will be a deep dive into all the forms and phases associated with advance care planning, and how to encourage advance care planning in your community. This webinar is intended to promote understanding of different advance care planning tools and resources that help with advance care planning. It will also provide background on the public policy landscape in which advance care planning sits today and discuss potential future changes and updates.

Objectives:

  • Understand and distinguish between health care proxy, health care directive, POLST/MOLST, and other advance care planning tools and documents
  • Gather ideas on how to encourage planning in your community
  • Access to resources that can help with ACP
  • Understand the federal policy landscape for advance care planning including debate on its utility

Corporate and campus risk managers, compliance officers, administrators, medical directors, PCPs, directors of nursing, mid-level providers, social workers, pastoral care, ethics committee and QAPI Committee members
