Root Cause Analysis (RCA) is a powerful tool that informs performance improvement, drives purposeful change to prevent harm, and improves outcomes for all stakeholders. With a focus on organization-wide system improvements, root cause analysis results in operationalizing efficient and reliable care and service delivery processes, which results in better financial sustainability.

This webinar will discuss the role of RCAs and how they fit into integrated risk management and QAPI activities. We will illustrate the steps and components of the RCA process and discuss how to leverage an RCA to create effective performance improvement recommendations and projects.

Objectives:

• Learn the role of RCAs and how they fit into integrated risk management and QAPI activities
• Discuss the evolution of separating out post-incident investigations (fact-finding) and performance improvement activities like RCAs
• Illustrate the steps and components of the RCA process
• Recognize the core members of an effective RCA team (e.g. administration, medical director/physician, interdepartmental staff, frontline staff) and the role they play in decreasing risk, improving quality, and increasing safety
• Define event types, problems, and the effect they have on identifying root causes

Intended Audience: Corporate and campus risk managers, compliance officers, administrators, medical directors, PCPs, directors of nursing, mid-level providers, infection preventionist, and QAPI Committee members

About the Speakers:

Jean Harpel, MSN, RN, GCNS is a geriatric clinical nurse specialist and a risk management analyst. Ms. Harpel is presently the Operations Manager of the Aging Services team at ECRI Institute where she provides environment-of-care safety and security and quality and risk management consultation to aging services/post-acute care providers. She educates leadership and staff on how to implement Risk Management systems within their communities. As part of the ECRI Institute consultation services she reviews incident reports from aging services member providers and works with the organizations to help reduce future risk and harm to their residents as well as helping achieve focus on developing a culture of safety and accountability. She has developed institutional- based learning programs utilizing electronic media, traditional lecture, just-in-time activities and bedside initiatives that have focused on safe and efficient care delivery. She has written, implemented and reviewed policies and procedures for continuing care retirement communities and home care providers.

Jeraldine Stoltzfus, MBA-HA, RN, BA is a patient safety analyst and consultant for ECRI. Her professional experience includes nursing, care management, quality improvement, risk management and patient safety. She started her professional nursing career working in acute care. She transitioned to out-patient services, charged with building the care management department as part of the patient centered medical home. In this role she developed a community clinical resource database, patient education, care plans, and standard operating procedures. Following this experience, she was a practice manager and director of quality within a Federal qualified health center. Jeraldine’s education includes a BA, Registered Nurse and a Master’s in Business Administration with a concentration in Healthcare Management.

Celena Romero, PhD, MBA, CPHQ, RD, LDN is a senior risk manager consultant for aging services at ECRI. Dr. Romero’s career spans over twenty years working in various healthcare environments as a Registered Dietitian and a Certified Professional in Healthcare Quality. Past work experience includes root cause analysis evaluation and facilitation, on-site and web-based risk assessments, in-person and remote educational seminars, personalized consultation, coordinating government and commercial pay for value/improvement programs, patient safety event data analysis, medical record and mortality review, quality improvement project collaboration, and healthcare award/recognition programs. Dr. Romero served as a dietetic internship preceptor, clinical nutrition manager, and a clinical nutrition support clinician. She completed a PhD and MBA in Healthcare Administration and conducted research focused on the impact of palliative care on patients hospitalized with severe chronic obstructive pulmonary disease.
